Dietary Needs & Restrictions

If your child requires a special dietary accommodation prescribed by a doctor that would require a special menu or substitution for your child, the Special Dietary Needs Form must be filled out. This form needs to be returned to the Nutrition Services Department at your school building or can be emailed to nsoffice@stcs.org If you have already filled one out for your child for a prior school year and there are no changes, you do not need to fill out another form.
